It's That time of year again

Yes, you have all been waiting for this, it’s Physical Time. Occupational Health will be emailing you the annual paperwork for you to fill out and return to them directly. Please watch your emails. These forms must be returned by May 19th, do not procrastinate. If you have never had one with us, you need to complete All tabs in the document.

 These Physicals are Mandatory for ALL Personnel. If you are full-time at another Department, you are exempt. I will need a copy of your 2025 fit for duty letter.

I will be posting the sign ups soon. There are two portions to sign up for, both must be completed.

The first portion of the Annual Physical is the Lab work and Screening. You are to fast for 10 hours prior to the Lab work. (water and black coffee is acceptable) This is Blood and Urine so plan accordingly. PSA will be offered to Males over 40. Mammograms are available to females over 40. (June 2-6th)

The second portion of the Physical is meeting with the Doctor. The Doctor will review your Labs and perform a physical evaluation. It is at the discretion of the doctor to require a Chest x-ray, or Treadmill.  (June 9-12th)

Please be careful when you sign up. Sign up in the open slots underneath the times posted.

If you can not make these dates please contact me immediately.

If you had a physical in 2025 at Occ Health please see me to see if you will need to complete a physical again.
