Hello AFD!

I wanted to pass along some important information shared by Brianna Kuechle regarding a new program at Northwestern Medicine Lake Forest. Starting November 6th, the Lake Forest Hospital at Home program will be available within Region X. This initiative provides eligible patients the option to receive hospital-level care in their own homes, allowing for continuous monitoring and support through both in-person visits and advanced remote technology.

Here’s what to keep in mind:

• Identifying Patients in the Program: Patients enrolled in the Hospital at Home program will have signage visible at their homes and wear an identification bracelet indicating their participation. These markers help EMS providers quickly recognize the patients who are part of this program.

• Transport to Lake Forest Hospital: If we encounter a patient who is enrolled in this program and a 911 transport is required, please prioritize Lake Forest Hospital as the destination whenever possible, per our protocols and transport criteria. This ensures continuity of care within the program’s structure.

• Region 10 SOGs Compliance: The region’s Standard Operating Procedures will continue to guide us in treating these patients. Unless directed otherwise by Region X protocols, patients enrolled in Hospital at Home who call 911 should ideally be transported back to Lake Forest Hospital.
